File 1 – Application Form and Agreement
File 2 – The Designs and Quality-Assurance Checklist duly completed for Design1, Design 2 and Quality Assurance
File 3 – Design 1 – Please ensure that you have referred to the pages of this document in the checklist.
File 4 – Design 2 – Please ensure that you have referred to the pages of this document in the checklist.
File 5 – Company Quality Assurance System – Please ensure that you have referred to the pages of this document in the checklist.
Please ensure all documents and attachments are well labelled. Applications must be submitted electronically by: e-mail or dropbox. Please note: incomplete applications will not be accepted.
Your application will be acknowledged by reply email, within three working days of your submission – if you do not receive this please contact firstname.lastname@example.org or phone (03) 341 2225.
An electronic invoice will then be issued. Your application fee must be paid up front. Your application will start being processed as soon as payment is made.
To ensure robust outcomes, three panel members will assess your application (a separate panel member for each design and the QA system).
During the assessment process you may be contacted and asked to clarify or provide additional information.
A decision will be made within three months of the assessment starting date. You will be advised by email and a formal letter. Successful applicants will receive a certificate of accreditation.
